CEU Card Replacement

NOTE: The RMATA did NOT offer ANY CEU events in 2021.

If you have lost your CEU confirmation card from a previous RMATA Annual Clinical Symposium by policy the RMATA requires a $15 re-issue fee. Please complete the request below.  If it is found that you did attend the event, the RMATA will re-issue the CEU confirmation card(s).
